Start with the mental model
Specific phobia matters because the fear attaches to one clearly identifiable trigger, and the blood-injection-injury subtype uniquely causes fainting rather than the racing heart of typical anxiety. It is diagnosed as marked, persistent fear of a specific object or situation, such as spiders, heights or injections, that is actively avoided or endured with intense anxiety. For example, imagine a patient who feels calm in every setting except when a blood test is mentioned, at which point she becomes pale and light-headed rather than simply anxious. In practice, that fainting response is the specific clue pointing to the blood-injection-injury subtype.

Separate the look-alikes
Most specific phobias cause a typical anxiety response, a racing heart and rising blood pressure, when the feared object or situation is encountered. The blood-injection-injury subtype is different: it produces an initial brief rise in heart rate followed by a vasovagal (a reflex causing a sudden drop in heart rate and blood pressure) response, causing dizziness or fainting. Applied tension, deliberately tensing the large muscles to raise blood pressure just before or during exposure, is a specific technique used to prevent this fainting during treatment. Say a patient practises applied tension in the waiting room just before a scheduled vaccination, tensing her arms and legs to keep her blood pressure from dropping. In practice, this makes exposure-based treatment safe to attempt even for a phobia that risks fainting.

Blood-injection-injury phobia

Agoraphobia
